Case Study in Evidentiary Maturity: Taking an AI-Based Clinical Decision Support System from Retrospective Validation to EHR Integration

Clinical AI tools often stall between promising retrospective results and routine use. Join Dr. Ashwin Rao as he discusses Scope-Pilot, a large language model-based decision-support tool for colonoscopy surveillance intervals, and its progression through successive stages of evidence development and implementation.

Dr. Rao will highlight retrospective validation using independent gold-standard datasets, a think-aloud study with practicing endoscopists to evaluate feasibility, acceptability and display design, and FHIR-based integration into Epic test environments at Baylor College of Medicine and UTHealth Houston.

The presentation also will outline a planned pragmatic evaluation examining effectiveness, implementation and human-AI discordance. Throughout the session, Dr. Rao will share practical lessons and decision-making strategies that can be applied to other clinical AI projects.
